Hockanum Valley Community Council, Inc. is a private non-profit social service agency located in the town of Vernon Connecticut.

We are a state licensed Outpatient Mental Health and Substance Abuse Clinic offering a wide range of counseling services and group work. We serve adults, adolescents and children (ages 7 and up) in the towns of Vernon, Ellington, Tolland and the surrounding area. We are available for any clients experiencing emotional, behavioral, or substance-related disorders with individual, family, or group therapy, as well as medication management.

Intake and Evaluation

  • For your convenience, you may now download the initial intake form and fill it out ahead of time. Please print the entire form (10 pages) and read it carefully. Complete and sign the first six (6) pages and bring them with you to your first appointment. (Pages 7-10 are for your records only.)   • The Intake Meeting or Substance Abuse/Mental Health Evaluation is the initial meeting with clients seeking services at HVCC. It is an assessment of the presenting problem and the client's family, medical, mental health, substance abuse, and legal history.
  • Based on the information provided by the client at Intake, treatment recommendations are made and approved by the Director of Counseling.
  • Clients will be informed of their treatment assignment within one week.
  • Referrals to Psychiatric Evaluations are made once the client is established (usually after three sessions of therapy). However, if a referral is made from a higher level of care, a referral for Psychiatric Evaluation may be made at Intake. Efforts are made to expedite Psychiatric appointments for emergencies.
  • If the initial evaluation indicates a need for a more appropriate level of care, then referrals to other treatment programs in the area will be made. In this situation, you may expect a phone call or letter from the Intake Clinician to inform you of these treatment recommendations. In addition, he or she will assist you in the referral process.
